Position can be located in Aberdeen, Sioux Falls or Pierre, SD. Must possess current SD license & certification as registered dietician/licensed nutritionist. Must have knowledge of food & nutrition sciences & nutrition education and chart documentation. Requires skills and abilities to maintain confidentiality programs, meet critical deadlines, speak/present to individuals and groups, manage time effectively, work with medical diabetes team and focus on patient outcome care. Will maintain daily contact with clients and families diagnosed as having diabetes, metabolic disorders, food & drug interactions, or other conditions complicated by diet. Will complete nutrition assessment and provide education & counseling and develop or adjust special diets. Will serve on the Diabetes/Healthy Heart Education team by assisting with planning & implementing activities, analyzing cost and care reports and completing midyear and annual audit information. Position will monitor nutrition services in three clinic settings (travel involved) for continuity and consistency when working with target population.
Must be aggressive with program follow-up to maintain participation rates and operate under criteria to meet grant requirements. Must have high proficiency and experience with EHR systems and must be extremely proficient with using computer systems. Knowledgeable of Native American culture and traditions preferred.
