Call to Freedom is committed to identifying and responding to the needs of individuals at-risk of human trafficking and/or sexual exploitation and those working to transition out of the life of human trafficking. The Bilingual Case Manager’s purpose is to identify needs, implement strategies, and coordinate individualized services for victims and survivors. This position will work with teams to manage tasks. Key tasks include educating and connecting with the foreign national community and managing the tracking and delivery process to referral partners, conducting supportive intake and screening processes, facilitating referrals, and providing case management.
REQUIRED EDUCATION AND/OR EXPERIENCE:
Prefer Bachelor’s degree in Social Work or Counseling. Experience in case management with victims of violence, trafficking, trauma, and/or substance abuse essential.
Training in trauma-informed care beneficial.
Excellent verbal and written communication skills. Flexible, self-directed, and responsive.
Bilingual – preferably Spanish/English.
This position is 40 hours/week hourly. Must be able to adapt to flexible scheduling including some nights and weekends.
CERTIFICATION, LICENSURE AND/OR REGISTRATIONS:
The ability to obtain and retain a valid driver’s license and motor vehicle insurance.
ESSENTIAL FUNCTIONS AND JOB DUTIES:
Develop, maintain, and track partnerships with other multi-cultural organizations within the community to collaborate on referrals and best client care.
Conduct initial screening and assessments working with individuals recognizing needs for safety, autonomy, and confidentiality.
Be a primary point of contact for assigned clients. Provide responsive and timely service to individuals.
Conduct trainings to multi-cultural and bilingual community and service providers on prevention curriculum and other community partner trainings.
Teamwork is an essential part of this position. Being able to work with different personalities and positions is vital.
Incorporate CTF’s Continuum of Care (CoC) model into case management practices to ensure seamless support, from a trafficking situation to long-term stability and wraparound services.
Provide outreach and technical assistance to community agencies to aid in identifying and assisting.
Develop and execute outreach clinics in collaboration with Statewide Rural Case Manager to underserved and rural populations.
Travel for agencies using personal/agency vehicles.
Attend weekly care conferences, staff meetings, luncheons, and other outreach events as assigned.
Maintain documentation and records appropriately and timely.
Responsible for daily entry of client data as needed into CTF database system.
Adhere to confidentiality policies regarding clients, HIPAA, and privacy.
Assist all case managers and occupational therapists working with bilingual clients with interpretation and supportive services.
Other duties as assigned.
