The Bright Start Nurse Home Visiting program provides education, support, resource coordination, and screenings to first time pregnant women and families. This full-time site nurse team lead position is responsible for monitoring and supporting the team's implementation of nurse home visiting. The local team may be comprised of Office of Home Visiting staff and/or registered nurses employed by partners who are contracted to provide Bright Start Services. Bright Start implements the Nurse Family Partnership (NFP) evidence based model, this position will report to the Bright Start Nurse Manager.
Duties of the position include:
- provide work direction to the Bright Start Team located in Sioux Falls and the surrounding area;
- ensure all staff attend and complete required Nurse Family Partnership (NFP) education as outlined in the NFP training manual;
- provide day-to-day guidance to local home visiting team by leading team meetings and providing reflective supervision to nurse team members;
- work with the Nurse Family Partnership (NFP) National Change Office to ensure that the model is implemented with quality and fidelity;
- assign client caseloads and monitor nurse home visitors ability to build and manage caseloads;
- develop and maintain internal and external systems to assure timely receipt and disposition of referrals;
- monitor local team reports on program outcomes, client demographics, and staff productivity;
- complete quality assurance activities such as joint visits, chart audits, and data assessments;
- build and maintain strong community partnerships with local non-profits, social service agencies, and other networks of support for home visiting families through presentations and relationship building. Participate in, chairs, or coordinates community advisory board meetings;
- participate in Bright Start Team Lead and other Regional meetings (as needed) to provide local team updates and receive training and technical assistance;
- travel regularly to local team sites for program oversight, staff support, onboarding/program orientation and periodic training for staff;
- provide coverage for nurse home visitors during staff vacancies or leave;
- travel to in-state locations for program meetings, and some out-of-state travel for trainings is required;
- other assignments may be added as needed or required.
Licenses and Certifications: Must be licensed or be eligible to be licensed as a Registered Nurse in South Dakota, with a bachelor's degree or higher required.
The Ideal Candidate Will Have:- worked in public or community health, including home visiting;
- have a background in maternal child health or perinatal health, particularly in a community setting.;
- program management and outreach experience.
Knowledge, Skills, and Abilities: Knowledge of:
- motivational interviewing;
- coaching;
- reflective practice.
Skill to:
- interpret program data and apply data to continuous quality improvement practices.
Ability to: - work both autonomously and as a contributing and supportive member of a nurse home visiting team;
- travel to multiple in-state locations;
- build the confidence and skill of nurse home visitors;
- assist nurses in analyzing complex problems and case situations.
This position is eligible for Veterans' Preference per ARSD 55:10:02:08.
