West River Mental Health is accepting applications for a Case Manager Position with our Full Circle program. The Case Manager Position assists clients in meeting the case management needs and goals that support their recovery during and after treatment at Full Circle.

ESSENTIAL JOB FUNCTIONS:
- Lead various life skills groups to help clients gain confidence with daily living activities such as employment, planning, budgeting, health, nutrition, cleaning, transportation, medication management, and self-care.
- Conduct individual intake sessions to identify and understand client’s needs and goals.
- Work with clients and staff to develop and implement person-centered case management plans.
- Conduct ongoing sessions to assess and track progress, educate clients on community resources, and facilitate aftercare planning.
- Locate support groups for clients, families, and/or significant others.
- Cultivate positive working relationships with a variety of community resources.
- Develop and maintain community resource lists.
- Maintain a safe and secure environment to promote a recovery-based environment atmosphere.
- Attend team meetings to collaborate and coordinate client care and provide key updates throughout the week.
- Complete all required documentation in an accurate and timely manner.
- Attend required training for continuous growth and improvement.
- Comply with BMS code of ethics, standards, policies, and HIPAA requirements.
- May be required to transport clients.
- Performs other related duties as assigned by management.
KNOWLEDGE, SKILLS, AND ABILITIES:
- General knowledge of substance use issues or willingness to develop an understanding.
- Knowledge of and skill in teaching basis life skills.
- Ability to interact professionally with clients and both internal and external customers.
- Ability to work successfully as part of a team to include demonstrating respect and collaboration.
- Ability to listen effectively, remain calm, and set boundaries.
- Ability to maintain a South Dakota driver’s license, vehicle liability insurance, and safely operate a vehicle in all weather conditions.
- Ability to respond to clients by phone and face to face in emergency situations.
EDUCATION AND EXPERIENCE:
Minimum of a High School Diploma or equivalent with preference for experience in a human services field. Associate’s degree in the social sciences or human services field or a Bachelor’s degree in a human service field and experience preferred.
