Join Our Team as a Case Manager (Reentry)
Call to Freedom is committed to identifying and responding to the needs of individuals at-risk of human trafficking and/or exploitation and those working to transition out of the life of human trafficking. The case manager’s purpose is to identify needs, implement strategies, and coordinate individualized services for victims and survivors in the prison systems, while assisting them through the reentry process. Key tasks include conducting supportive intake and screening processes, facilitating referrals, and providing case management alongside prison staff and procedures.
REQUIRED EDUCATION AND/OR EXPERIENCE:
Bachelor’s degree in Social Work, Counseling or related human services field. Experience in case management with victims of violence, trafficking, trauma, substance abuse, corrections field, and mental health is essential.
Training in trauma-informed care is beneficial.
Excellent verbal and written communication skills. Flexible, self-directed, self-starter, and responsive.
Approximately 40 hours/week, must be able to adapt to flexible scheduling; including evening and weekend hours, as needed.
CERTIFICATION, LICENSURE AND/OR REGISTRATIONS:
- The ability to obtain and retain a valid driver’s license and motor vehicle insurance.
ESSENTIAL FUNCTIONS AND JOB DUTIES:
Be a primary point of contact for assigned clients. Provide responsive and timely service to individuals by offering case management and supportive services.
Conduct initial screenings and assessments and work with individuals, recognizing needs for safety, autonomy, and confidentiality. Conduct follow-up calls and videos with inmates regarding eligibility and case management for those incarcerated.
Using trauma-informed best practices, work with assigned clients to develop and implement individualized case management goals and positive reentry plans.
Maintain documentation and records as appropriate.
Work with community partners to build referral sources and collaborations.
Responsible for daily entry of client data on the database system.
Travels for the agency using a personal/agency vehicle.
Attends staff meetings, luncheons and other outreach events as assigned.
Maintain a minimum of bi-weekly contact and/or weekly appointments for clients, based upon their status (incarceration, community, outreach, etc.).
Work with Call to Freedom’s Occupational Therapists, Spirituality & Mentor Coordinator, Family Development Case Manager, or Survivor Service & Program Coordinator as needed for clients.
Adhere to confidentiality policy regarding clients.
Continue to develop own self-knowledge of issues regarding human trafficking.
Work with Manager of Youth & Reentry Case Managers to assign and get clearance for appropriate mental health providers.
Ensure all materials are fully stocked on an ongoing basis for inmate access at facilities.
Facilitate trainings - possible trainings in correctional facilities.
Other duties as assigned.

REQUIRED PHYSICAL ABILITIES:
- While performing the duties of this job, the employee is regularly required to stand and sit; use hands to handle or feel (keyboard, use copy machine, etc.) and talk or hear.
- The employee is required to move about and reach with hands and arms; stoop, kneel, crouch (Reach for supplies on shelves at shoulder height or above).
- The employee is required to sit and occasionally climb or balance (Balance while assisting moving supplies. Climb a flight of 2-6 steps).
- The employee must be able to view computer screens for long periods of time (50% or more of the time for the position is computer related or brief periods of time).
- The employee must regularly lift and/or move about up to 10 pounds and occasionally lift and/or move about up to 30 pounds (Lift copy paper or supplies from one level to another.)
Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
