Position Overview:Children’s Home Society of South Dakota is seeking a Case Manager to provide in-home case management services and appropriate interventions to children and families in accordance with the organization’s mission, vision, and values. What You Will Do:Case Management
- Provide case management services to children and youth, to include home visits, community navigation, family contacts/visitation, referral for services within the community, transportation, and other needs as identified through case planning process.
- Complete evidenced based assessments during case plan evaluation process for child and/or caregiver.

- Must be able to provide courteous and respectful personal attention to promote individual client satisfaction and the public perception and reputation of the Agency.

- Respond effectively, tactfully, and with maturity in highly-emotionally- charged situations.

Communication Methods:
- Must be able to get important points across in the process of responding to children, adult clients, and co-workers.
- Listen carefully to complex issues in order to effectively respond.
- Portrays a welcoming and accepting attitude to clients, co-workers, and the public. Difficult and sensitive issues require careful articulation so as to preserve a relationship or to prevent misunderstanding and potential liability to the Agency.

- Cooperatively and respectfully interacts with other team members to encourage positive results. Work with therapeutic and clinical staff in referrals into case management program and address workflow process needs.
Data/Records Management:
- Complete records and documentation per Agency Policy, licensing, and accreditation standards.
- Must be able to maintain the highest standards of confidentiality for children, youth, and their caregivers.
- Maintain records in a confidential manner. Receive information from justice system and therapeutic partners and maintain partner standards of confidentiality.

Requirements
Education:- Bachelor’s Degree required, preferably with emphasis in Social Work or related field.
- Master’s degree in social work or similar field is preferred.
- Must be willing to obtain ongoing training as required through the agency and specific to the role.
Experience: - Minimum two years’ experience working with children and families. Knowledge of normal child development is required
- Experience in crisis stabilization for children, youth, and families.
Certifications/Licensure:- Must have CHW Certificate, or ability to obtain certificate.
- Social Work Associate or higher licensure preferred but not required.
Physical Demands and Work Environment:Normal office environment with minimal exposure to dust, noise, etc. Extended PC viewing and keyboarding required. Extended periods of sedentary work required. Client home visits are required, which may include evening hours.
