Clinical Care Coordinator (RN) — Home Instead® | Watertown, SD
Full-Time | Monday–Friday | 8:00am–5:00pm
Office Location: 810 South Maple, Watertown, SD 57201

Home Instead is seeking a compassionate and motivated Clinical Care Coordinator to support seniors and families throughout Northeastern South Dakota—including Watertown, Brookings, and Aberdeen. This position plays a key role in ensuring our clients receive exceptional care that keeps them safe, happy, and independent in their homes.

What You’ll Do:

  • Conduct in-home client assessments and develop individualized care plans

  • Provide ongoing clinical direction, support, and training to caregiving staff

  • Maintain compliance with state regulations and quality standards

  • Collaborate daily with the office team to coordinate services and support client needs

  • Participate in an on-call rotation (typically once per month) with additional on-call pay

Qualifications:

  • Current Registered Nurse (RN) license in South Dakota

  • Strong communication, assessment, and leadership skills

  • A passion for helping older adults remain independent at home
