RESPONSIBILITIES: As part of the LSS team, the Case Manager helps develop individualized case plans, conducts ongoing assessments, and connects youth with appropriate community resources. This position is essential in identifying the needs of youth and their families, supporting their growth, and fostering positive change in their lives. Success in this role requires strong communication and organizational skills, along with a deep commitment to helping others. Responsibilities include maintaining accurate case documentation; building collaborative relationships with community partners and service providers; coordinating medical, dental, and vision appointments; ensuring annual exams are completed; supporting medication management processes in collaboration with nursing staff and pharmacy providers; obtaining medical records and treatment consents; reviewing Medication Administration Records (MARs) for accuracy; and transporting clients to appointments as needed.
QUALIFICATIONS: Candidates must possess or be eligible to obtain licensure as a Social Worker or Social Work Associate through the South Dakota Board of Social Work Examiners, with in 6 months of employment. Obtaining and maintaining licensure is a requirement of this position. Guidance through the licensure process will be provided by Lutheran Social Services.
- A bachelor’s degree in social work or a related field, is required.
- Must be at least 21 years of age, is required.
- Licensed Social Worker or Licensed Social Work Associate through the South Dakota Board of Social Work Examiners is preferred.
- Experience with client Medication Administration Records (MARs), is preferred.
- Willingness to submit to record checks to detect criminal convictions of behavior harmful towards children, spousal abuse, distributing drugs or sex crimes.
- Requires a valid auto-insurance, a valid driver’s license or the ability to obtain one with approved driver status.
