Job Summary
Under the general supervision of the Director of Pharmacy, following all policies and regulations, the pharmacist will be responsible for accurately interpreting orders, entering orders in EMR, verifying orders in EMR, compounding and dispensing pharmaceuticals, and monitoring drug therapy regimens for appropriate dosing, appropriate frequency, contraindications, interactions, allergies, and duplications. The Pharmacist provides pharmacokinetics, dosing calculations, therapy recommendations, and research drug information. The pharmacist interacts with members of the health care team to provide safe and effective care. The pharmacist assists, supervises, and verifies tasks completed by pharmacy technicians as needed.?

- Interprets medication orders (verbal, written, electronic) and transcribes accurately and/or verifies pharmaceutical orders in the EMR. Reviews patient profile with verification for appropriateness, allergies, interactions, incompatibilities, assesses age-related dosage, and dosage frequency.
- Maintains accurate complete medication profiles. Adapts positively to interruptions and changes in workload.
- Provides pharmaceutical kinetic service, clinical consultation, clarification to medical providers and staff. Suggests appropriate cost-effective therapeutic alternatives to medical providers as needed. Maintains the pharmacy formulary by minimizing nonformulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy. Maintains proper levels of medications to meet patient needs.
- Recommends changes to medication therapy based on laboratory findings.
- Participates in interdisciplinary patient care rounding, promptly contacts medical providers with questions or concerns of orders. Utilizes pharmaceutical references knowledgeably to provide accurate information.
- Compounds and dispenses pharmaceuticals, including non-sterile compounded products, sterile compounded products, and hazardous compounded products accurately and safely. Practices proper safety techniques in accordance with hospital and departmental safety policies and procedures, and state/federal regulations including USP 795 Non-sterile compounding, USP 797 Sterile compounding, and USP 800 Storing and Handing Hazardous Pharmaceuticals.
- Assures that all pharmaceuticals which are out of date, lost potency, or recalled have been removed from inventory and processed as a return or pharmaceutical waste. Disposes of contaminated materials appropriately and pharmaceutical waste in accordance with hospital and departmental policies and procedures.
- Issues controlled substances to patient care areas and maintains records as required by law.
- Demonstrates sound professional judgement consistent with clinical academic background as in assessing situation, considering alternatives, choosing the appropriate course of action.
Other Job Duties:
- Reviews literature and other materials pertinent to the practice of pharmacy. Provides accurate, adequate and timely drug information to medical practitioners, staff, patients, and family.
- Participates in hospital quality, risk, and utilization plans. Follows infection control procedures and universal precautions as applicable to work as established by policy.? Follows quality improvement by demonstrating competency in appropriate IV techniques for sterile compounded products. Maintains data logs, medication dispensing records and other documentation as required.
- Participates in department meetings, in-services, educational programs, and mandatory meetings. Communicates congenially and professionally with staff, patients, and visitors.
- Demonstrates the ability to address problems using tools and techniques for identification and resolution. Works independently with minimal supervision. Organizes and prioritizes work. Ensures pharmacy services are provided in a timely manner. Documents required activities and interventions accurately and completely.
- Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. participates in the performance evaluation of pharmacy staff.
- Is punctual and dependable with tardiness. Maintains a neat and professional appearance.
- Answers department phone by identifying self and department. Directs calls to appropriate personnel.
- Fosters a team environment, reports positive suggestions within the scope of the department.
- Participates in the quality improvement, medication usage audits, and review of department activities. Performs all tasks or duties as assigned.
