As a Community Health Worker (CHW), you'll empower individuals enrolled in the CHOICES waiver program by helping them navigate the healthcare system, access vital community resources, achieve personal health goals, and maintain their independence. What You'll Do
As a Community Health Worker, you'll serve as a trusted partner and advocate by helping participants navigate healthcare services, coordinate appointments, connect with community resources, and achieve their individual health goals. You'll provide health education and wellness coaching, collaborate with healthcare providers and case managers, accompany participants to medical appointments, and support personalized care plans that promote healthier, more independent living.
You'll also document services accurately to meet Medicaid requirements while ensuring every participant receives compassionate, person-centered support.

- No Community Health Worker certification? No problem! If you're not already certified, LifeScape will provide the required training and cover the cost of certification within your first 90 days of employment (Contingent on acceptance into program and passing fingerprinting requirements)
