POSITION TITLE: Dental Care Liaison
LOCATION: Sioux Falls, SD
DEPARTMENT: Foundation
REPORTS TO: Program Manager
STATUS: Non-exempt (hourly)
OVERVIEW OF POSITION:
The Dental Care Liaison’s primary role is to connect children who have been identified with oral health needs to dentists who have committed to providing care. This position ensures that vulnerable children overcome barriers to accessing oral health services by providing care coordination, education, and advocacy. The Liaison will work closely with families, schools, dental providers, and community partners to ensure children receive timely and appropriate care.
CARE COORDINATION
Build and maintain relationships with dental providers willing to serve uninsured or underinsured children.
Partner with school nurses, teachers, and community agencies to identify children in need of dental care and the appropriate pathway for them to receive care.
Assist families in scheduling and attending dental appointments, including addressing barriers such as transportation, childcare, language services, and if needed, pre-surgical physicals.
Support children and families in follow-up care, reinforcing treatment plans and preventive practices.
Screen patients for insurance eligibility.
EDUCATION AND ADVOCACY
Educate children and families about the importance of oral health and its connection to overall well-being.
Provide group presentations in schools, community organizations, and outreach events.
Advocate for patients with dental providers, community health centers, and other service agencies.
COMMUNITY OUTREACH
Conduct outreach through schools, WIC, Head Start, and other community-based programs to promote oral health and identify children with unmet dental needs.
Collaborate with medical providers and social service agencies to support families in addressing related health or social needs.
PROGRAM SUPPORT
Document patient interactions and provider partnerships in accordance with program requirements.
Assist with planning and implementing oral health events and initiatives in dental offices and the community.
Attend community health events, trainings, and related meetings
Contribute to the Foundation’s short- and long-term goals.
EXPERIENCE AND QUALIFICATIONS:
The candidate must be passionate, persistent, detail-oriented, adaptable, and a self-starter with strong communication and problem-solving skills.
Prior experience in dental services, care coordination, or working with children and families is preferred.
Associate’s degree or certificate in social work, human services, dental hygiene, dental assisting, public health or a related field; equivalent experience considered.
Previous experience in care coordination, case management, or dental/healthcare services preferred.
Strong interpersonal and communication skills (oral and written).
Proficiency with basic computer applications.
Ability to adapt to changing needs and work independently or as part of a team.
Valid driver’s license and reliable transportation required.
Conversational Spanish (or other language skills) preferred.

This position will be full time hourly position. Starting salary is $23.08/hr ($48,000/yr).
