POSITION SUMMARY
The Family Coach/Case Manager at our inpatient addiction recovery facility for pregnant and parenting women provides comprehensive individual and family support, crisis management, intensive case management, and parenting skills education to mothers. This role involves offering a range of services, including finding daycare, managing Child Protective Services (CPS) involvement, accessing Women Infant and Children (WIC) services, securing child support and childcare assistance, facilitating GED education, and maintaining employment. In collaboration with counselors, the Family Coach/Case Manager assists clients in working through their Individualized Service Plans, addressing both short- and long-term goals, objectives, progress, and milestones. Professionalism is paramount in all interactions with children and their mothers, as well as in all duties performed.
Providing regular, intensive case management, the Family Coach/Case Manager manages CPS and legal involvement, Department of Social Services (DSS) paperwork, finds employment and housing, obtains identification documents, and connects clients with other community resources as needed. The Family Coach/Case Manager will work with CPS to support clients wanting to reunite with their children by navigating the social services systems and steps to achieve outlined goals. Additionally, this role supports clients in accessing services for their children once reunification is achieved like childcare and birth-to-three services. This position will assist clients in complying with legal involvement by communicating progress with their team and supporting clients with required activities. Guided by the Volunteers of America, Dakotas Cultural Code, the Family Coach/Case Manager ensures that services are delivered with empathy, dignity, and care, promoting a supportive and empowering environment.

ESSENTIAL FUNCTIONS STATEMENT(S)
A. Conduct screenings and assessments at intake, throughout the program and at discharge.
B. Provide regular, intensive case management to clients to include but not limited to CPS, DSS, and legal paperwork, finding employment, housing, obtaining identification documents, and other community resources as needed.
C. Develop and maintain a working relationship with referral sources.
D. Assist with data collection, including timely documentation of client services. Complete weekly documentation notes for each client.
E. Attend training required to meet program licensing and accreditation standards.
F. Follow up with clients after successful discharge for required amount of time.
G. Assist with birth to 3 services for children.
H. Support educational and school registrations.
I. Assist in caring for children when necessary.
J. Ensure client files are in compliance with standards within the case manager scope.
K. Attend weekly staff meetings as scheduled.
L. Meet individually with clients on a regular basis.
M. Prep, prepare and teach parenting group from established curriculum.
N. Provide transportation for appointments as needed.
Responsible for other duties as assigned.
POSITIONS QUALIFICATIONS SKILLS & ABILITIES
Education: Bachelor’s degree in social work or related human services field required.
Experience: A minimum of two years related work experience or an equivalent combination or education and experience preferred. Must have experience and working knowledge of case management planning and community interconnection.
Skills: Excellent written and verbal communication skills. Ability to manage multiple priorities and balance perspectives. Able to remain calm in escalated situations. Effective communication, appropriate boundaries, flexibility and willingness to learn are essential. Familiarity with American Indian and/or other minority cultures preferred.
Other Requirements: Must pass all applicable background checks. Must have valid driver’s license and insurance due to client transportation needs.
PHYSICAL DEMANDS
A. Ability to Stand, Walk, sit, manually manipulate, reach outward and above shoulders, speak, climb, crawl, squat, kneel, and bend on a daily basis
B. Capability to Lift, carry, push or pull up to 25 lbs on a daily basis
