FLSA Status:          Non Exempt                                                          Reports To:  Program Nurse Educator

Position Supervises:   NA

Work Schedule:   Position requires flexible hours. With some evening and weekend transportation for hospital admissions/discharges.

Have a commitment to supporting participants recovering from substance use disorder and assist them with their overall health while at the New Start Program

ESSENTIAL FUNCTIONS STATEMENT(S)

  • Work as part of the New Start’s interdisciplinary team to help ensure the medical needs of women and children are met and ensure continuity of care.
  • Facilitate completion of medical service-related activities, including medical appointments and diagnostic activities.
  • Assist in scheduling medical appointments.
  • Provide transportation to appointments.
  • Accompany participants and their children to medical appointments to:
    •  Explain contents of medical visit paperwork to participant and nurse
    •  Ensure prescriptions, prescription changes, and ordered treatments or dressing changes are sent to contracted pharmacy.
    • Support clients as active participants in their own care by encouraging clients to ask clarifying questions, request written explanations and education.
  • Support participants with accessing and using various patient portal platforms
    • Assist participant with setting up patient portal access inf not already established
    • Assist client with printing relevant lab results, etc. for transfer to New Start client record
    • Support participants in communicating return/upcoming appointments to their New Start team.
  • Organize data in reviewable report formats.
  • Have current Medication aide Certification or complete Medication Aide online curriculum within 90 days of position start date.
  • Administer medications as needed.
  • Ensure program compliance with licensing, accreditation standards, health and safety requirements.
  • Follow and use the A-TEAM Cultural Code.
  • Ensure safe working conditions of equipment.
  • Ensure compliance with health, fire and safety codes.
  • Ensure communication of facility needs to appropriate administrative staff.

POSITIONS QUALIFICATIONS
SKILLS & ABILITIES

Education: High School Diploma or GED equivalent

Experience: Some medical knowledge and residential treatment preferred.

Skills: Have a passion for helping other. Written and oral communication skills necessary. Ability to prioritize and multitask. Familiarity with electronic medical records and patient portal platforms a plus but not required. Possess the ability to use judgment and maintain professional relationships with participants and community entities.

Certifications & Licenses:

Other Requirements: Must have valid driver’s license and insurance. Must have the ability to be insured by the VOAD’s insurance carrier.

PHYSICAL DEMANDS

  1. Ability to Stand, Walk, sit, manually manipulate, reach outward and above shoulders, speak, climb, crawl, squat, kneel, and bend on a daily basis
  2. Capability to Lift, carry, push or pull up to 100 lbs on a daily basis
