The South Dakota Department of Health is hiring a full-time Health Facilities Surveyor to assist with protecting and promoting the health and safety of residents and patients in South Dakota's licensed health care facilities. The Health Facilities Surveyor is responsible for surveying patient activities, dietary, infection control, resident rights, and physical environment. The Surveyor's role is to ensure compliance with state and federal regulations for the protection of health and safety hazards. This position requires travel, offers a daily per diem and the use of a state vehicle while conducting surveys.

Working from home is a privilege and is subject to review depending upon the position. The selected candidate will have to travel to one of the main office locations for in office days which are located in Rapid City, Pierre, Mitchell, and Sioux Falls.

Travel and overnight stays are required for this position.

Licenses and Certifications:
The candidate must be licensed related to their professional role, if applicable. Training for the survey process is through required online Centers for Medicare and Medicaid Services (CMS) courses and through precepted surveys in licensed healthcare facilities

The Ideal Candidate Will Have:
Experience working in a health care facility/service. Understand the standards of quality care. The ability to learn and clearly convey information to others and check for understanding, sense and understand others' concerns and feelings, and develop self to expand own knowledge, apply it, and share with others.

Knowledge of:

  • principles and practices of the disciplines providing services to patients and residents in health care facilities;
  • medical terminology;
  • the causes and control of diseases produced by unsanitary conditions.


Skill to:

  • exercise sound judgment and make timely, well-informed decisions;
  • set goals and put forth the effort required to achieve them;
  • conduct work in an honest, ethical, responsible, and committed manner;
  • plan ahead, set priorities, and work in an orderly manner;

  • review and assess effectiveness or compliance of multi-disciplinary care interventions and progress towards meeting outcomes of care; revises the plan of care or identify corrective measures for compliance or to prevent the spread of disease and illnesses;
  • provide technical assistance and information that contributes to the development of wider solutions or approaches to healthcare issues and situations;
  • conduct or review ongoing psychosocial, physical, and/or functional nursing assessments of individuals/families/populations to determine a plan of action/intervention;
  • conduct ongoing assessments of the care delivery processes of a particular healthcare population to promote change that will support desired outcomes;
  • review records, practices, and/or operations to assess the quality of care and to ensure appropriate inter-disciplinary communication for the continuity of care;
  • provide orientation and mentors/preceptors to junior colleagues, interns, and/or students to develop and prepare quality practitioners.


Ability to:

  • access all occupied areas of a health care facility to conduct inspections and complete the survey process.

This position is eligible for Veterans' Preference per ARSD 55:10:02:08.
