If you are passionate about helping the elderly maintain their independence, we have a great opportunity for a full-time position.
The HUD Service Coordinator is responsible for building informal support networks with residents, family, and friends of tenants. They will provide case management with the participating residents to ensure grant qualifications are met. The HUD Service Coordinator will work as a liaison, building relationships with community partners that provide programming for residents.
ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.
1. Observe given office hours.
2. Educate staff on issues related to aging in place.
3. Provide basic case management and referral services.
4. Assist residents in securing transportation for appointments.
5. Assist residents in securing utility assistance.
6. Assist residents in securing assistance with meals on wheels.
7. Develop a resource directory for residents.
8. Facilitate community partnerships within the complex as well as outside agencies.
9. Complete documentation process to meet grant requirements.
10. Conduct research on services provided and used per resident.
11. Organize programs on topics of interest to residents.
12. Share and coordinate enrollment in recreational programs offer in the city for the residents.

-Previous experience working with the elderly.
-Demonstrated working knowledge of supportive services and other resources in the area.
-Demonstrate your ability to advocate, organize, problem solve, and provide results for the residents.
-Exhibit excellent oral and written communication skills.
-Willingness to work independently while staying on task.
-Pristine attention to detail.
-Must be punctual and dependable.
-Must be able to provide proof of eligibility to work in the United States.
-Ability to work 40 hours per week.
-Minimum of 2 years’ experience working in a supportive services role.
-Willingness to use personal vehicle for work purposes.
Wage range: $17.50 - $18.50 per hour
