The Human Services Social Worker is an important and valuable member to the South Dakota Veterans Home by "taking care of America's heroes" by assessing and providing direct social services to meet the needs of residents and their families in the long-term care setting. These social services include, but are not limited to: discharge planning, collateral contact management, serving as a member of the admissions team, the advanced care planning team, and complaint committee; and intervenes in the patient's out-of-hospital environment to serve as the primary formal link between the patient, family, and community support systems, working collaboratively with the interdisciplinary team to provide exceptional care for residents and their spouses.
Onsite, low-cost housing may be also be available; cost is dependent on square footage.
Licenses and Certifications: The Human Services Social Worker must have a baccalaureate degree as a Social Worker or nonsocial work field or discipline or an associate of arts degree in human services in a program.
The Ideal Candidate Will Have: The Ideal Candidate will have social work licensure or the ability to become a licensed social worker. Experience in related field and/or long-term care experience is also preferred.
Knowledge of:
- ethical standards, principles, and practices of social work, counseling, and human behavior;
- long term social work requirements;
- social and psychosocial assessments including the MDS Assessment;
- current social, economic, and health trends and services, and the ability to adapt trends to practical program applications and link clients with services needed;
- interviewing and other information gathering techniques;
- assessment principles and techniques;
- available community resources;
- record keeping;
- chemical dependency.
Skill to:
- develop and execute discharge plans to ensure the resident's well-being and security;
- compile and prepare accurate social histories, previous treatment information and patient assessments and share findings with other members of the interdisciplinary team to determine effective treatment and therapies to meet resident needs and goals;
- consult with resident's family and/or guardian to inform them of the patients progress and to promote the resident's best interest;
- establish and maintain individual patient records to ensure accurate, up to date information;
- conduct individual, group, and family counseling and resolves conflicts;
- provide ongoing treatment, case management, and rehabilitation services to residents with severe and/or persistent mental illnesses.
Ability to:
- remain calm with difficult patients and individuals;
- develop cooperative and satisfactory contacts with clients, patients, relatives, physicians, social agencies, community officials, and other administrative officials;
- create meaningful, concise, and accurate reports and correspondence;
- establish and maintain effective working relationships with diverse populations;
- communicate effectively with a wide variety of people;
- assess social service needs, complete social assessments and social histories, and identify and obtain programs or services sufficient to meet these needs;
- obtain reliable information.
This position is eligible for Veterans' Preference per ARSD 55:10:02:08.
