Department: Marissa's House | Reports to: Director of Marissa's House and Outreach Service

Summary

Call to Freedom is committed to identifying and responding to the needs of individuals at-risk of human trafficking and/or exploitation and those working to transition out of the life of human trafficking. The Case Manager’s purpose is to identify needs, implement strategies, and coordinate individualized services for victims and survivors. Key tasks include conducting supportive intake and screening processes, facilitating referrals, and providing case management.

Key Responsibilities

  • Be a primary point of contact for assigned clients. Provide responsive and timely service to individuals.
  • Conduct initial screenings and assessments, recognizing needs for safety, autonomy, and confidentiality for individuals.
  • Work with Marissa’s House and Outreach clients to develop and implement individualized case management goals and plans and providing appropriate resources which could include but is not limited to: mentorship, employment, housing, therapy, medical care, and/or basic needs/documentation (ID’s, social, birth certificate, etc.)
  • Work alongside the Occupational Therapy team, Spirituality Coordinator, Family Development Case manager, MH case manager, and Survivor Service and Program Coordinator to facilitate case planning, goals, and house groups.
  • Assist in screening and interviewing potential new Marissa’s House referrals.
  • Maintain documentation and records as appropriate including yearly case audit files.
  • Represent Marissa's House at quarterly volunteer orientations.
  • Work with Marissa’s House team to develop house schedules week by week and post them to MH Van/House schedule and scheduling the monthly house meeting.
  • Help clients move in and out of apartments as needed.
  • Implement occupational therapy plan formed with the Occupational Therapist to carry out individual client goals at Marissa’s House, as necessary.
  • Assist in scheduling volunteers for Marissa’s House including but not limited to ground keeping, cleaning, and organization of donations.
  • Ability to drive or learn to drive a 12-passenger van in all weather conditions in order to transport clients and their children when appropriate.
  • Work with team to maintain a healthy, clean, positive “home” environment.
  • Work with Spirituality Coordinator in pairing clients with mentors.
  • Understand and enforce alarm system and respond if alarm is activated as needed for secondary response.
  • Provide training to community partners, service providers, public groups, schools etc. as requested to increase awareness, identification, report, and referral of victims for services.
  • Organize, track, and manage MH rent, MH scholarships applications, and rental assistance as needed.
  • Organize survivor’s support groups/programming with MH team for MH clients.
  • Other duties as assigned.

Required Education & Experience

  • Prefer Bachelor’s degree in Social Work or Counseling. Experience in case management with victims of violence, trafficking, trauma, and/or substance abuse essential.
  • Training in trauma-informed care beneficial.
  • Excellent verbal and written communication skills. Flexible, self-directed, and responsive.

Certification, Licensure and/or Registrations

  • The ability to obtain and retain a valid driver's license and motor vehicle insurance.

Required Physical Abilities

  • While performing the duties of this job, the employee is regularly required to stand and sit; use hands to handle or feel (keyboard, use copy machine, etc.) and talk or hear.
  • The employee is required to move about and reach with hands and arms; stoop, kneel, crouch (Reach for supplies on shelves at shoulder height or above).
  • The employee is required to sit and occasionally climb or balance (Balance while assisting moving supplies. Climb a flight of 2-6 steps).
  • The employee must be able to view computer screens for long periods of time (50% or more of the time for the position is computer related or brief periods of time).
  • The employee must regularly lift and/or move about up to 10 pounds and occasionally lift and/or move about up to 30 pounds (Lift copy paper or supplies from one level to another.)
  • Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Required Communication/Mathematical/Reasoning Skills

  • Must possess the required level of skills and ability to read, write, speak and apply mathematical concepts and sounds reasoning to successfully perform the essential duties and responsibilities of this job.
  • Must be able to perform advanced operations on a computer within the Windows operating system.
  • This position requires problem solving skills, critical thinking skills and the ability to multi-task and focus on specific tasks.

Schedule & Environment

  • Full-time (40+ hours/week) with flexible scheduling
  • Some evenings and occasional weekend hours required
