Job Summary: The Nurse Consultant is responsible for conducting routine Minimum Data Set (MDS) validation reviews to ensure the accuracy of Medicaid reimbursement for long term care facilities. The position provides oversight across a broad region of central South Dakota, including Walworth, Potter, Faulk, Spink, Sully, Hyde, Hand, Beadle, Kingsbury, Hughes, Buffalo, Jerauld, Sanborn, Miner, Brule, Aurora, Davison, Hanson, and McCook counties.
In this role, the nurse consultant evaluates comprehensive clinical documentation to determine the appropriate level of care for Medicaid long term care residents and Home and Community Based Services (HCBS) participants. Responsibilities also include supporting the administration of the Pre Admission Screening and Resident Review (PASRR) process, specifically by completing level of care determinations for individuals with serious mental illness triggers who are seeking nursing facility placement.
Additionally, the position provides ongoing education, guidance, and technical assistance to nursing facility staff. The nurse consultant works collaboratively with providers to promote regulatory compliance, strengthen documentation accuracy, and ensure individuals receive services in the most appropriate and least restrictive setting.
Very limited travel may be required within the designated coverage area.
Influenza vaccination required.
Licenses and Certifications: Candidate must be licensed to practice as a Registered Nurse in South Dakota and have a valid driver's license.
The Ideal Candidate Will Have: Knowledge:
- knowledge of cultural, economic, social, physical, and psychological factors that influence older adults and adults with disabilities' health care decisions;
- minimum Data Set (MDS) assessment experience in the long-term care setting (preferred but not required);
- current Registered Assessment Coordinator certification or willingness to become certified within first 6 months of employment.
Skills:
- skills to prepare clear and concise documentation, reports, and correspondence that reflects pertinent findings.
Abilities:
- gather and analyze data to accurately determine appropriate level of care;
- review and make timely decisions based on medical documentation;
- utilize a computerized database system for accurate record keeping;
- comprehend, interpret, and apply pertinent laws, regulations, and procedures to specific determinations;
- communicate clear and concise information through verbal and written correspondence;
- establish and maintain effective working relationships with long term care providers;
- make accurate and timely decisions and assume accountability for outcomes;
- plan and organize work to meet schedules and deadlines while responding and adjusting to multiple demands and shifting priorities;
- conduct infrequent travel within coverage area if necessary as well as for training opportunities.
