Call to Freedom is looking for a strong self-starter to work in our new Rapid City office!

Must be comfortable working independently and willing to travel within the Rapid City and surrounding area as well as to the Sioux Falls office throughout the year as needed.

Call to Freedom is committed to identifying and responding to the needs of individuals at-risk of human trafficking and/or exploitation and those working to transition out of the life of human trafficking. The Outreach Case Manager’s purpose is to identify needs, implement strategies, and coordinate individualized services for victims and survivors. Key tasks include conducting supportive intake and screening processes, facilitating referrals, and providing case management.

REQUIRED EDUCATION AND/OR EXPERIENCE:

  • Prefer a bachelor's degree in social work or counseling. Experience in case management with victims of violence, trafficking, trauma, and/or substance abuse is essential.

  • Training in trauma-informed care is beneficial.

  • Excellent verbal and written communication skills. Flexible, self-directed, and responsive.

  • This position is 40 hours/week hourly. Must be able to adapt to flexible scheduling including evening and weekend hours as needed.

CERTIFICATION, LICENSURE AND/OR REGISTRATIONS:

  • The ability to obtain and retain a valid driver’s license and motor vehicle insurance.

ESSENTIAL FUNCTIONS AND JOB DUTIES:

  • Be a primary point of contact for assigned clients. Provide responsive and timely service to individuals.

  • Conduct initial screenings and assessments and work with individuals, recognizing needs for safety, autonomy, and confidentiality.

  • Prepare and implement assessments and intake tools for CTF and community partners to utilize.

  • Work with youth and adult clients to develop and implement individualized case management goals and plans.

  • Maintain documentation and records as appropriate.

  • Teamwork is an essential part of this position. Being able to work with different personalities and positions is vital.

  • Ability to self-manage, work independently, and stay productive in a remote office setting.

  • Incorporate CTF’s Continuum of Care (CoC) model into case management practices to ensure seamless support, from a trafficking situation to long-term stability and wraparound services.

  • Work with local and rural community partners to build referral sources, collaborations, and resources for clients.

  • Responsible for daily entry of client data on database system.

  • Responsible for transportation of clients using personal or company vehicle when appropriate.

  • Provide education and conduct trainings to community partners when appropriate.

  • Learn and implement prevention curriculum within the community and surrounding areas.

  • Participate in collaborative care meetings within the community.

  • Work with referral sources who provide services to newly identified victims, and be the main point of contact between CTF and the referral source/rural response teams.

  • Work with Director of Marissa’s House & Outreach Services and Manager of Youth & Reentry Case Managers to receive new referrals via CTF Intake Line and navigate/assign cases as appropriate.

  • Work alongside the care team at CTF (OT, Spirituality & Mentor Coordinator, Housing Navigator, Survivor Service & Program Coordinator, etc.) to provide direct services to clients either in person or virtually.

  • Travel as needed for CTF to meet with community response teams, partner organizations, and new connections using agency provided vehicles. This can include overnight trips.

  • Attend weekly care conferences, staff meetings, and other outreach events as assigned. This includes virtual meetings and some in-person meetings requiring travel to the Sioux Falls office throughout the year.

  • Adhere to confidentiality policies regarding clients, HIPAA, and privacy.

  • Conduct ongoing trainings to service partners, and attend trainings (in person or webinar) regarding human trafficking (labor and sex) or trauma-informed practices.

  • Other duties as assigned.
