RESPONSIBILITIES: The Outreach Case Manager works within the Arise Youth Center, which serves as an alternative to youth detention. The Center provides services for young people who are involved in the juvenile justice system, are runaway or homeless, or are at risk of becoming so. In this role, the Case Manager partners closely with youth and their families to offer support, coordinate services, and help clients work toward their personal goals. As part of the LSS team, the Case Manager helps develop individualized case plans, conducts ongoing assessments, and connects youth with appropriate community resources. This position is essential in identifying the needs of youth and their families, supporting their growth, and fostering positive change in their lives. Success in this role requires strong communication and organizational skills, along with a deep commitment to helping others.
QUALIFICATIONS:
- Bachelor’s degree and one year of experience in a human services environment, or an equivalent combination of post-secondary education and relevant experience.
- Must be at least 21 years of age, is required.
- Willingness to submit to record checks to detect criminal convictions of behavior harmful towards children, spousal abuse, distributing drugs or sex crimes.
- Requires a valid auto-insurance, a valid driver’s license or the ability to obtain one with approved driver status.
