Job Description
The City of Sioux Falls Health Department is accepting applications for a part-time Community Health Worker at Falls Community Health. This position is a maximum of 28 hours per week, M-F, with flexible schedule based on the needs of the clinic.
Community Health Workers work in clinical, community-based and home settings to provide care coordination and connection to resources and support to patients to improve their health and general well-being through education and coordinated care and services.
Responsibilities included in this position are as follows:
- Provide community- and clinic-based HIV testing and counseling.
- Provide PrEP education and navigation, including helping individuals understand PrEP.
- Educate community members about HIV prevention, PrEP, routine HIV testing, and the importance of knowing your HIV status.
- Participate in community outreach events, health fairs, testing events, and other HIV prevention activities.
- Provide linkage to care and follow-up support for individuals who receive a positive HIV test or need additional services.
- Build relationships with community organizations and other partners to expand awareness of and access to HIV prevention services.
- Promote HIV awareness and stigma reduction through culturally responsive, nonjudgmental education and outreach.
- Engage individuals who may not routinely access traditional healthcare settings and connect them with appropriate testing, prevention, and care resources.
The selected candidate will receive training to work within a clinic setting, as well as information and guidance to build foundational knowledge in HIV testing, counseling and PrEP. In addition, the selected candidate will connect with community members involved in HIV treatment and develop an understanding of resources and services available in the community.
Qualifications and Skills:
Graduation from high school or GED certification with one (1) year experience in a clinical or social services setting; or any such combination of education, experience, and training as may be acceptable to the hiring authority.
Completion of an approved State of South Dakota Community Health Worker certificate program is required within one year of hire.
