Position Description:

Hours: Monday-Friday, 8am-5pm

Center for Family Medicine is looking to add a grant-funded position of Peer Recovery Navigator to our healthcare team. The Peer Recovery Navigator serves as a vital member of an integrated primary care team, helping patients with substance use disorders and other complex behavioral health needs successfully navigate healthcare and community services. Working within a family medicine clinic, this position bridges medical care, behavioral health, hospitals, addiction treatment providers, and community agencies to improve engagement, reduce barriers to care, and support long-term recovery.

Required Skills:
1. Personal lived experience in recovery from substance use disorder or experience supporting family members in recovery.
2. Experience working with individuals affected by substance use disorders, mental illness, homelessness, or other vulnerable populations.
3. Knowledge of community resources and healthcare systems.
4. Excellent interpersonal and communication skills.
5. Ability to work independently while functioning effectively within an interdisciplinary healthcare team.
6. Experience with motivational interviewing and trauma-informed care.
7. Familiarity with medications for opioid use disorder (MOUD), harm reduction principles, and recovery-oriented systems of care, preferred.
8. Experience using electronic health records.

Educational Requirements:
1. High school diploma or GED required.
2. Certification as a Community Health Worker or ability to obtain certification within two months.
3. Certified Peer Recovery Specialist/Peer Support Specialist (or eligible for certification within one month), preferred.
4. Associate's or Bachelor's degree in social work, psychology, public health, human services, or related field, preferred.
5. Experience working in primary care, integrated behavioral health, emergency medicine, or addiction treatment, preferred.
