Job Description
Peer Support Specialist will work in a team atmosphere providing services to adults with Severe and Persistent Mental Illness. Peer support specialists share their own lived experiences with mental illness and what skills, strengths, supports, and resources they use and will share their own recovery stories and will demonstrate how they have directed their own recovery processes in order to help clients achieve treatment goals.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Work includes identification and outreach, needs assessment, service plan development, and service mobilization, linkage and monitoring.
- Assist clients to obtain needed service, supports, and entitlements by consulting with service providers, locating available resources, and assisting clients to complete forms, make appointments, and secure transportation.
- Assesses client’s needs by gathering relevant information from individual contacts with clients, reviewing available records, and securing information from other mental health agencies and persons familiar with client’s case.
- Continuously expand knowledge of community resources, services, and programs available to clients and build ongoing relationships with these organizations to advocate for clients.
- Help clients understand and employ recovery strategies
- Engage clients to achieve health management goals through the use of recovery coaching, motivational interviewing, and problem-solving techniques.
REQUIREMENTS: High school diploma or GED, valid driver's license.
