Reentry & Rural Case Manager

Mission-driven role supporting survivors of human trafficking
As a Reentry & Rural Case Manager at Call to Freedom, you will walk alongside individuals transitioning out of exploitation and incarceration—connecting them to safety, support, and resources that help them move toward independence and healing.
At Call to Freedom, we are committed to identifying and responding to the needs of individuals at risk of human trafficking and those working to transition out of exploitation. This role plays a vital part in supporting survivors preparing for reentry into the community and ensuring they have access to the services and support systems they need to succeed.
What You'll Do
- Provide trauma-informed case management for survivors of trafficking and exploitation
- Conduct intake screenings, assessments, and ongoing case management
- Develop individualized service plans with youth and adult clients
- Coordinate services with correctional facilities, rural response teams, shelters, halfway houses, and other community partners
- Conduct phone, video, and in-person visits with incarcerated and community-based clients
- Maintain documentation and enter client data in the database system
- Assist with transportation for clients when appropriate
- Provide education and training to community partners on human trafficking and trauma-informed care
- Participate in collaborative care meetings and community outreach efforts

What You'll Need
Bachelor’s degree in Social Work, Counseling, or related field preferred
Experience in case management with victims of violence, trafficking, trauma, substance abuse, or mental health challenges
Experience working with correctional settings preferred
Training in trauma-informed care is beneficial
Valid driver’s license and ability to maintain motor vehicle insurance required
Ability to travel throughout the region up to 3–4 days per week (depending on future growth), including occasional overnight trips
Flexibility for occasional evening or weekend hours as needed
Additional Requirements
- Ability to sit at a computer for extended periods
- Ability to occasionally lift up to 30 pounds
