Position Description
Provides health care for individuals with mental illness by assessing needs and implementing appropriate nursing interventions of patients, determining if a physician’s attention is necessary and following orders given by a physician/Certified Nurse Practitioner to ensure patient health care needs are met. Services provided in the community (client’s homes) and in the clinic setting.
Responsibilities for this position include:
- Completes comprehensive patient assessment and formulates or updates nursing care plans
- Assesses the patient’s physical condition
- Conducts periodic patient interviews
- Provides comprehensive patient care according to the individual’s treatment plan
- Conducts group and individual activities appropriate to treatment plans
- Administers medications and treatments as prescribed by a physician/CNP to ensure patients’ medical needs are addressed and side effects from medications and treatments are documented
- Monitors patients’ response to medications and treatments and relays data to the physician/CNP
- Explains prescribed medications and treatments to patients and staff
- Maintains patient records to maintain adequate and accurate patient data
- Records pertinent medical or behavioral data
- Completes special reports and forms
- Provides basic medical care, determines if physician/CNP’s care is needed and necessary, and make arrangement for obtaining it
- Evaluates and documents specific health problems of patients, determines what action is necessary and administers medications as appropriate
- Assesses the physical needs of patients, observes patients for illness or injury, and answers questions of staff regarding behavior of patients
- Provides input during treatment team meetings to develop a comprehensive treatment plan for each patient
- Ability to collaborate with healthcare providers for continuity of care
- Is familiar with the Suicide Safe Care protocol and takes the proper steps if concerned about the personal safety of a client
Requirements
- License Practical Nurse or Registered Nurse in South Dakota
- Experience in mental health preferred
- Knowledgeable in current nursing theory and practice
- Skills in crisis and emergency management
- Ability to work as part of a team
- Excellent computer skills/proficient in computer applications, specifically MS Office
- Ability to multi-task and prioritize work
- Problem solving skills
- Excellent communication skills, both verbal and written
- Strong organizational skills
- Ability to meet deadlines
- Possess a valid driver’s license
