Job Description

Job Summary

We are seeking a compassionate, dedicated Registered Nurse (RN) to join our CARE Program team. In this role, you will play a critical part in supporting adult clients living with Severe Mental Illness (SMI) in the community, as well as collaborating closely with our case management team. You will conduct health assessments, oversee medication management, coordinate medical care, and empower clients to achieve independence and wellness in their home environments.

Key Responsibilities

  • Health Support: Perform physical and psychiatric nursing assessments for adult clients enrolled in the CARE program.

  • Medication Administration & Oversight: Monitor medication compliance, manage prescription refills, administer injections (e.g., long-acting injectables), and educate clients on side effects and adherence.

  • Case Management Collaboration: Partner closely with case managers, therapists, and psychiatrists to develop, review, and execute individual care plans. Oversee medication administration/set-ups administered by casemanagers, including training casemanagers in appropriate medication set ups.

  • Community-Based Care: Conduct home visits and community check-ins to monitor clients' overall health, self-care routines, and living conditions.

  • Crisis Intervention & Triage: Identify early signs of psychiatric decompensation or physical medical crises and initiate appropriate interventions or emergency referrals.

  • Health Education: Provide one-on-one and group health education on chronic disease management, wellness, hygiene, and nutrition.

  • Documentation: Maintain accurate, timely clinical documentation in compliance with state regulations and agency standards.

Qualifications & Requirements

  • Education: Graduate of an accredited School of Nursing (ASN or BSN).

  • Licensure: Active Registered Nurse (RN) license in the State of South Dakota (or a valid Compact/eNLC license with SD privilege).

  • Experience:

    • Minimum of 1 year of clinical nursing experience required.

    • Experience in psychiatric/mental health nursing, community health, or case management strongly preferred.

  • Skills: Strong communication, active listening, conflict resolution, and clinical judgment skills. Empathy and understanding when working with populations with complex psychiatric and social needs.

  • Other Requirements: Valid driver's license, reliable transportation, auto insurance, and ability to pass a criminal background check.
