DESCRIPTION

Position Summary


 The Shared/Family Living Specialist position holds the responsibility of supporting health, safety and growth of the individuals in a Shared Living program that incorporates host families, consumer owned homes, independent contractors and strong involvement with biological families as applicable. Job duties will include integration into the community, skill building, addressing programmatic concerns, incident reporting and adhering to Administrative Rules of South Dakota (ARSD). A commitment to RHD values should be demonstrated as job duties are performed.


Essential Duties and Functions



  • Create an environment that empowers individuals (both individuals, independent contractors, and all other team members) through the development of skills and coordination of resources and supportive services.

  • Oversee the quality of programmatic plans, community access, volunteer opportunities, work experiences, and necessary services and supports that are provided to meet the individuals’ needs and preferences.

  • Advocate for the individual’s needs.

  • Represent RHD Shared Living program in a professional manner and model leadership and professional boundaries at all times.

  • Protect all HIPAA related information on a need to know basis.

  • Attend and document ISP meetings, and ISP reviews along with CFCM. Monitor annual ISP for progress towards goals and supports for all individuals. Ensure all ISPs are implemented and assessments have been completed.

  • Complete daily evaluation and documentation in Avatar, enter BSP and ISP into Avatar as needed.

  • Assist individuals in determining what type of living situation, outcomes, volunteer opportunities, and work experiences would meet their needs and preferences.

  • Assist in creating, reviewing, and implementing individual outcomes, action steps, progress notes and additional data sheets to monitor individual progress.

  • Reconcile individual money with Shared Living Providers.

  • Help recruit and maintain Shared Living Providers.

  • Ensure proper money ledgers, receipts, attendance logs, provider billing are send to FADMIN in a timely manner.

  • Train and document Shared Living Providers in how to implement ISPs, BSPs, and outcomes as needed.

  • Complete site visits with the individual present three times per month or more often as needed.

  • Complete all paperwork with Shared Living Provider, as well as completing annual home inspection evaluation.

  • Attend, assist in the planning of and actively participate in all team meetings.

  • Develop, write, present and train restrictive procedure plans and BSPs.

  • Ensure that Shared Living Providers receive in-service training or any additional training that is needed in conjunction with the SD Trainer.

  • Organize, maintain and update consumer files in accordance with ARSD regulations and program requirements.

  • Maintain tracking system to ensure files are updated as required, and conduct self-audits on required documentation.

  • Complete SMART file reviews as required by the Division of Developmental Disabilities.

  • Enter data and maintain Avatar and Therap.

  • Complete and review annual assessments and distribute to the ISP team in partner with CFCM.

  • Complete and review with the individual and ISP team assessments, BSP, RR and other related documentation.

  • Notify Director of any billing issues as soon as identified. Correct and document all billing issues and address with the SLP.

  • Ensure that all service delivery forms, progress notes, data sheets and other forms as required by regulations, RHD, Division or program are completed and updated in a timely manner and accurately to reflect the individual’s needs.

  • Maintain open and professional communication with individuals, families, Shared Living Providers, legal guardians, other agency staff, work or day programs, medical team, and all other outside entities.

  • Coordinate and attend monitoring visits of CFCMs and other outside monitors.

  • Participate in all program audits completed by the Team Leader, Director, or other RHD agency personnel.

  • Be an active participant in interviewing and choosing new potential Shared Living Providers as well as assist in the coordination and implementation of the contract procedure.

  • Attend as an active participant in supervisory meetings with the Director in order to obtain clear feedback, directions, training or problem resolution strategies.

  • Serve an incident management point person, determine if an incident is reportable and accurately complete Incident Reporting forms. Communicate details of incidents with the Investigator/Director and assist in scheduling investigative interviews as needed.

  • Coordinate visits to homes for potential new intakes.

  • Coordinate all admissions and discharges of individuals in conjunction with Team Leader. Be an active participant in all transition planning.

  • Ability to be available for rotating “on-call” calendar.

  • Complete all other miscellaneous duties as requested or assigned.


 


 

REQUIREMENTS
Qualifications

• Minimum requirement: Bachelor’s degree in human services field and minimum of 1 year working directly with individuals with Intellectual/Developmental Disabilities.
• Other preferred qualifications include:
o Knowledge of Administrative Rules of South Dakota, CHOICES Waiver
o Demonstrate strong professionalism, organizational, leadership, flexibility and time management skills
o Demonstrate the ability to work independently with minimum of supervision.
o Ability to effectively communicate both verbally and written forms.
o Ability to support and implement RHDs Values as well as the mission and philosophy of Shared Living Program.
o Ability to engage individuals, Shared Living Providers, families/guardians in effective, therapeutic and healthy interactions.
o Strong computer skills
o Possess a valid SD Driver’s License and have reliable transportation
o Must pass criminal background check.
