Job Description
Custer Care & Rehab is seeking a caring individual to assist residents with their transition of moving into a skilled nursing facility as well as helping them with the social aspects of living in a long term care facility.
Functions include but are not limited to: Care plan creation, work with residents and families, social isolation, room choice and personal preferences with their room, admission/referral process management.
