HOURS: 40 hours week.

RESPONSIBILITIES: Provide comprehensive case management services by assessing client needs and developing individualized service plans that support children and families in achieving stability and well-being. Coordinate and connect clients with appropriate community resources including medical, educational, legal, financial, and behavioral health services while collaborating with referral partners and treatment teams. Offer basic counseling, support foster care placements and family visitation, monitor progress toward treatment goals, and advocate for clients to strengthen functioning and outcomes. Maintain accurate, timely documentation, reports, and case records in accordance with organizational and regulatory standards.
QUALIFICATIONS:
- A Bachelor’s degree in social work or a related field, plus one to two years related experience, is required.
- Licensed Social Worker or Social Work Associate (or the ability to be licensed) through the South Dakota Board of Social Work Examiners, is required.
- Must be at least 21 years of age, is required.
- Willingness to submit to record checks to detect criminal convictions of behavior harmful towards children, spousal abuse, distributing drugs or sex crimes.
- Requires a valid auto-insurance, a valid driver’s license or the ability to obtain one with approved driver status.
