Position Purpose:
Provides and coordinates psychosocial, counseling, care coordination and social services for patients and families at Huron Regional Medical Center across numerous departments including hospice and dialysis. Ensures timely assessments, regulatory compliance, interdisciplinary collaboration, and support that enhances patient well-being, adjustment, and quality of life.

Hospice Social Work Responsibilities
- Completes a psychosocial assessment within five (5) days of hospice admission.
- Provides counseling related to death, dying, anticipatory grief, and adjustment to terminal illness.
- Assesses bereavement risk factors and documents findings according to social work standards.
- Assessment of the relationship of the beneficiary's medical and nursing requirements to the beneficiary's home situation, financial resources and availability of community resource
- Appropriate action to obtain available community resources to assist in resolving the beneficiary's problem.
- Counseling services that are required by the patient.
- Participates in Interdisciplinary Group (IDG) care conferences and contributes to the development of the hospice plan of care.
- Assists in discharge planning when hospice care is no longer appropriate.
- Provides education and support to patients, families, and caregivers regarding hospice philosophy, coping strategies, and available resources.
- Assists in conducting psychosocial seminars for hospice personnel, patients, families, and the community.
- Assessing and addressing caregiving resources and needs.
Dialysis Social Work Responsibilities
- Completes the Initial Social Work Assessment within 30 days or 13 treatments of chronic hemodialysis initiation.
- Conducts a 90-day assessment, annual reassessments, and monthly reassessments for unstable patients.
- Evaluates:
- Cognitive status and understanding of treatment
- Ability to meet basic needs and follow treatment prescriptions
- Mental health history and counseling needs
- Substance use history
- Adjustment to kidney failure and dialysis
- Expectations for future functioning and quality of life
- Employment, education, and vocational concerns
- Home environment and support systems
- Legal issues (guardianship, advance directives, health care proxy)
- Housing needs, including advocacy for traditional and non-traditional settings
- Financial capabilities and eligibility for Federal, State, and local programs
- Access to community resources
- Completes the social work plan of care within 30 days (or 13 treatments).
- Provides counseling to support adjustment, adherence, and coping with chronic illness.
- Assists patients in achieving psychosocial and rehabilitation goals.
- Administers the KDQOL-36 quality-of-life tool at 90 days, annually, and as needed after major life events or health changes.
- Participates in the Interdisciplinary Team (IDT) with the patient/designee, dietician, nurse, and nephrologist.
- Collaborates with IDT members to identify and monitor unstable patients and adjust care accordingly.
Counseling, Support, and Crisis Response
- Provides individual and family counseling for emotional, social, or economic stressors.
- Responds to urgent or crisis situations to support patients, families, and staff.
- Offers education on treatment options, advance directives, and patient rights.
- Supports patients and families in navigating complex decisions related to chronic illness or end-of-life care.
Resource Navigation & Community Coordination
- Assesses financial needs and connects patients/families with appropriate resources.
- Maintains current knowledge of community programs, benefits, and support services.
- Acts as a resource regarding Federal, State, and private agency regulations.
- Builds and maintains relationships with community agencies to support patient needs.
Documentation & Compliance
- Documents all assessments, interventions, and communications accurately and in a timely manner.
- Ensures compliance with hospice and dialysis regulatory requirements.
- Participates in department quality, risk, and utilization review activities.
- Prepares reports and data as requested.
Other Job Duties:
- Participates in professional development, in-services, internal committees, and mandatory trainings.
- Completes required HealthStream education, competencies, certifications, and SQSS tasks.
- Works within the chain of command and performs other duties as assigned.
