ESSENTIAL FUNCTIONS & DUTIES

Student Support

  1. Assists with student supervision.
  2. Assists students with academic needs.
  3. Assists within the school store.
  4. Maintain confidentiality of students and families.
  5. Perform other duties as assigned.

504 Plan Facilitator

  1. Enters and maintains banks of student objectives and information for Individual the 504 Plan database.
  2. Assists the Building 504 Coordinator with the completion of the 504 Plan evaluation, which includes gathering relevant data, such as, parent input, medical documentation, educational documentation, etc.
  3. Assists with the dissemination 504 Plan information to identified teachers and administration
  4. Contacts parents to schedule 504 Plan meetings and conferences.
  5. Routes and maintains all paperwork required for compliance with 504 Plan regulations.
  6. Participates in all 504 Plan compliance training as requested by the District.

PAY INFORMATION

Pay Day: Last working day of each month.

HOURS & FTE

Monday through Friday, 8:00 am to 3:30 pm with a 30-minute unpaid break

0.875 FTE, 35 hours per week, 9 months

0.475 FTE as Student Support

0.4 FTE as 504 Plan Facilitator
