The primary role of this position will be to ensure the community based developmental disabilities system continues to progressively evolve with the changing needs of South Dakotans with ID/DD.
The Transition and Community Capacity Manager is responsible for supporting and enabling the placement and appropriate support of people with intellectual or developmental disabilities (ID/DD) transitioning settings, developing continuum of care capacity in the developmental disabilities service system to support people with ID/DD and complex support needs; identifying and developing systemic training opportunities; providing technical assistance, and implementing systems change efforts. This position will work directly with Community Support Providers, systemic partners, and stakeholders to assure the health, safety, and welfare of program participants.
Key Responsibilities:
- Independently engage with South Dakota Developmental Center (SDDC), the Human Services Center, Child Welfare and Community Support Providers to enable the placement of individuals transitioning from institutional care into community settings.
- Work with participants and providers to develop appropriate supports for individuals returning to community services from short-term hospitalizations.
- Work with providers to support the appropriate placement of and services for participants making life transitions, including youth aging into adult services, including those from child welfare settings.
- Partner with other agencies and divisions within DHS to enable appropriate services and planning supports are available.
- Serve as DDD lead for Money Follows the Person programs to ensure funds are reaching the providers and within federal guidelines.
- Engage with partners, stakeholders, families and providers to identify gaps in the existing continuum of care and develop strategies to build capacity in these areas.
- Work with the HCBS Program Administrator to support efforts to strengthen the provider network for DDD programs.
- Coordinate directly with SDDC for all crises care situations • Must understand the roles of other human services program specialists to ensure continuity of operations
This position requires some in-state travel.

- applicable federal and state laws, rules and regulations pertaining to developmental disabilities;

- work closely with other DDD employees to effectively and efficiently complete assigned work; and,
- assume initiative with minimal supervision.
