HOURS: Full-Time, 40 hours per week, occasional evenings as needed.

SALARY: Exempt. Dependent on experience and qualifications.

TRAVEL: Travel is required. Mileage reimbursement.

In this role, you will have the opportunity to strengthen community partnerships, support families through challenging circumstances, and help shape the continued growth and success of the Wraparound program.

A Day in the Life of a Wraparound Program Manager

As the Wraparound Program Manager, you will provide leadership, guidance, and oversight to the Wraparound/Family Stabilization team while helping ensure children, youth, and families receive coordinated, family-driven support.

Your day may include supporting staff with complex cases, co-facilitating Family Team Conferences, providing crisis consultation, and collaborating with community partners. You will also help guide the overall success of the program through strategic planning, budget oversight, program reporting, and identifying resources to support program sustainability.

You will build strong relationships with community and program partners, represent LSS in the community, and ensure staff have the support and guidance needed to successfully serve children, youth, and families. Occasional evening consultation and crisis stabilization support may also be needed.

RESPONSIBILITIES:

  • Provide information and intake services to families, referral sources, and community partners regarding Wraparound services and the C.A.R.E.S. model.
  • Facilitate Family Team weekly meetings and guide families through the Wraparound process, including developing individualized Plans of Care and Crisis Plans.
  • Assess family strengths, needs, resources, and circumstances to support individualized service planning.
  • Provide supportive counseling, resource coordination, and referrals to help children, youth, and families build on their strengths and access appropriate community and natural supports.
  • Maintain respectful and professional relationships with families, referral sources, and community partners while providing ongoing consultation and coordination of services.
  • Maintain timely and accurate documentation in required systems, including CareLogic, and complete required program data and reporting.
  • Represent LSS and the C.A.R.E.S. Wraparound program professionally, participate in required meetings and training.
  • Travel as required to meet with families, attend meetings, and coordinate services within the community.

QUALIFICATIONS: Candidates must possess or be eligible to obtain licensure as a Social Worker or Social Work Associate through the South Dakota Board of Social Work Examiners. Obtaining and maintaining licensure is a requirement of this position. Guidance through the licensure process will be provided by Lutheran Social Services of SD.

  • Bachelor’s degree in social work or related field, plus four years related experience is required.
  • Licensed Social Worker or Licensed Social Work Associate through the South Dakota Board of Social Work Examiners is preferred.
  • Must be at least 21 years of age, is required.
  • Must successfully complete all agency background checks, is required.
  • Requires active auto-insurance, a valid driver’s license or the ability to obtain one with approved driver status.
